Responsibilities - YOUR Role
- Promote a culture of Person and Family-Centred Care (PFCC) through education, engagement, and everyday actions.
- Ensure care plans are completed and maintained according to facility policies and regulatory requirements.
- Promote and maintain a strong culture of safety across the site, including emergency response readiness and workplace health and safety programs.
- Ensure compliance with all regulatory bodies, including the Ministry of Health, Health Authority, Community Care Licensing, and Accreditation Canada.
- Conduct regular audits on chart quality, hand hygiene, workplace violence prevention, and infection control, and lead improvements based on results.
- Deliver comprehensive education and training programs for staff, residents, and families, ensuring alignment with accreditation standards and best practices.
- Monitor infection rates and implement training and initiatives to improve outcomes.
- Review and analyze incident reports, identify trends, and lead risk management strategies.
- Lead compliance with immunization policies for both staff and residents.
- Support quality improvement initiatives by using data from MDS RAI assessments to inform decisions.
- Act promptly on safety, infection control, and care concerns in collaboration with leadership.

- Completion of an undergraduate degree in health sciences or related field. Post graduate courses and/or certification in Gerontology are an asset OR the equivalent in experience.
- Holds an active registration with the BCCNM as an LPN.
- A minimum of five years recent experience in gerontology with comprehensive knowledge of nursing and health care practices and techniques as they relate to long term care.
- Intermediate computer skills (including MS Office, specifically Outlook, Excel and Word).
- Proficiency in Health Informatics systems (e.g. Minimum Data Set (MDS), Gold Care or Point Click Care, eMar.)
- Proven time management skills including the ability to plan, set priorities and manage.
- Ability to problem solve interdependently and independently as required
