- Salary: $65,000 - $68,000 per annum
- Schedule: Tuesday - Friday, 8:30am - 4:30pm & Saturday 10am - 6pm

LPN Clinical Coordinator Must Haves:
- Graduation from a recognized Practical Nurses program or equivalent combination of education, training, and experience recognized by the BCCNM.
- Holds an active registration with the BCCNM as an LPN.
- Minimum 2 years of Long Term Care experience
- Maintain Occupational First Aide Level 1 status.
- Maintains a valid CPR “A” Certificate.
- RAI MDS education (Gold Care or Point Click Care).
LPN Clinical Coordinator Responsibilities:
- Provides committed leadership to the care department within the framework of the Mission, Vision, Values, and Philosophy of the Pro Vita as well as the Home.
- Develops and delivers training and education.
- Develops individual action plans and strategies to focus own performance towards the goals of the strategic plan.
- Formulates, monitors, and evaluates nursing practice to ensure quality professional care and standards of clinical practice comply with professional, legal and licensing requirements.
- Maintains broad internal and external contacts at a senior level for the purpose of enhancing the delivery of care programs and services for the organization.
- Collaborates with physicians, medical specialists, pharmacists, foot care nurse, dental hygienist, and other allied health professionals to ensure delivery of a continuum of integrated resident
- Performs Home centered audits as scheduled.
- Participates in discharge planning activities.
- Monitoring, ordering and stocking of care supplies.
- Works under the direction of the Director of Care to ensure Clinical KPI and care needs of residents are met
- Reports to the Care Manager for all human resource and employment related matters
- Works as unit nurse under emergent situations as needed
- Covers the Care Manager when on vacation to support in staff related concerns with the HR&Scheduling Coordinator
